Traumatized and suffering from severe amnesia after escaping a subterranean nightmare, Sarah Carter is pulled from the Appalachian cave system by local authorities, covered in the blood of her fallen friends. Sheriff’s Deputy Vaines and Deputy Rios refuse to believe her disjointed warnings about carnivorous, blind humanoids residing in the dark. Instead, they force Sarah to accompany a heavily armed rescue team back underground to find the five missing women. Alongside the skeptical lawmen and seasoned caver Rios, the group includes an aggressive U.S. Marshal named Rios, and two underground guides, Dan and Greg, who are eager to prove Sarah’s claims are nothing more than a psychotic hallucination.Descending back into the suffocating, claustrophobic tunnels, the group navigates tight spaces that immediately trigger Sarah's terrifying flashbacks. The tension escalates as internal conflicts divide the rescuers, particularly when they discover a severed hand, confirming Sarah’s dark narrative. Soon, the eerie, clicking sounds of the Crawlers echo through the abyss. The creatures attack with lethal precision, separating the team in the pitch-black labyrinth. Dan is brutally ambushed and dragged away, leaving the survivors realizing too late that the cave is a hunting ground and they are the prey.As the body count rises, Sarah regains her memories, realizing her past survival was just a temporary reprieve. Rather than cowering, her psyche shifts from a victim to a fierce predator, wielding improvised weapons with brutal efficiency. Vaines is severely injured, getting his leg trapped in a massive cave-in, and is ultimately abandoned by Marshal Rios in a selfish bid for self-preservation. Meanwhile, Sarah and Rios form an
uneasy alliance, relying on each other's wits to navigate the shifting tunnels and fend off relentless waves of the pale, flesh-eating humanoids. In a chaotic climax, Sarah, Rios, and Vaines—who is briefly rescued before being overwhelmed—fight their way toward a known exit shaft. During a violent confrontation on a subterranean cliff, Vaines sacrifices himself to drag the lead Crawler
into the abyss. Sarah and Rios finally break through to the surface, bursting out into the blinding sunlight and collapsing in exhaustion. However, the apparent rescue shatters instantly. The seemingly friendly driver of the transport truck turns out to be one of the subterranean guides in league with the local underground community, or simply a guardian of the dark. He strikes Rios unconscious, and Sarah is violently dragged backward, hurled back down into the dark bowels of the cave as the heavy grate slams shut above her.
