Years after the global outbreak engineered by the Umbrella Corporation, the Earth has been scorched into a desolate wasteland, and the deadly T-virus has turned humanity into hordes of the undead. Carlos Olivera and L.J. join forces with Claire Redfield, leading a nomadic convoy of survivors across the American desert in search of fuel, supplies, and safety. They are unaware that they are constantly monitored from afar. Deep underground in a Nevada facility, Dr. Isaacs continues his sinister work for Umbrella, obsessed with capturing Alice. Alice, having developed powerful psychokinetic and telepathic abilities as a result of her genetic mutations, wanders the ruined landscape alone. She keeps a low profile to protect her friends, knowing Umbrella's satellites can track her unique biological signature. Eventually, Claire’s convoy crosses paths with Alice. After Alice rescues them from an attack by mutated, hyper-aggressive crows using her newfound powers, she integrates into the group, learning of a rumored safe haven in Alaska called "Sanctuary." Desperate for a fresh start, the convoy heads north. However, Dr. Isaacs uses a tracking signal to locate Alice, unleashing a new breed of enhanced, intelligent zombies designed to overpower the survivors. During a brutal ambush at a motel, the convoy suffers heavy casualties, and L.J. is bitten, hiding the infection. When the zombies overrun the camp, a desperate battle ensues. L.J. turns and attacks the group, fatally biting Carlos before being put down. With their supplies dwindling and vehicles destroyed, the survivors realize they must secure the Umbrella facility nearby for transportation and fuel. Knowing the risks, Alice, Claire, and the remaining crew execute a daring raid on the underground installation. Inside, they discover Dr. Isaacs has mutated into a monstrous, grotesque super-creature after injecting himself with an overdose of Alice’s blood combined with antiviral agents. Carlos sacrifices himself by driving a fuel truck into the facility's entrance, blowing up the surrounding hordes of undead to buy the others time to escape. Alice faces Dr. Isaacs in a final, climactic showdown within the facility's simulated Tokyo laboratory. Utilizing her combat skills and newly honed mental powers, she manages to defeat him. Afterward, Alice accesses the facility's main computer and confronts the White Queen, the artificial intelligence who reveals that Alice's blood holds the key to curing the T-virus. Realizing that Umbrella's board of directors is still operating from underground complexes around the world, Alice looks into a secure camera feed and vows to bring the fight directly to them, rallying an army of cloned Alices to end Umbrella's reign of terror once and for all.
