Following the horrific events at her family home, young Kirsty Cotton finds herself confined to Channard Institute, a psychiatric hospital run by the eccentric and deeply obsessed Dr. Phillip Channard. Unlike traditional psychiatrists, Channard is secretly fascinated by the occult, specifically the Lament Configuration and the theology of the Labyrinth, a subterranean dimension ruled by Leviathan, the God of Suffering. Desperate to save her father and stop Channard, Kirsty enters the labyrinthine dimensions of hell, accompanied by Kyle. Inside this surreal, nightmarish realm of shifting corridors and infinite torment, they navigate various psychological and physical horrors. Kirsty eventually encounters her father, only to realize in a devastating twist that the spirit is actually a trick orchestrated by the evil forces, or that her father has already been claimed beyond salvation.As the chaos reaches its peak, Kirsty confronts both the resurrected Julia and the monstrously upgraded Dr. Channard. Kirsty uses a clever deception, showing Julia a photograph of Frank Cotton to remind her of her past betrayal and fatal greed. A furious Julia turns on Channard, but the sadistic doctor easily overpowers and kills her, draining her remaining life essence. Realizing that Tiffany is the key to closing the gate, Kirsty manages to free the terrified girl.
